On or about 3/17/2016 a) Employer did not provide safety inspection to ensure safe crane lift procedures were used for the type of steel beams being lifted; beams have offset center of gravity requiring rigging adjustment and have 8 inch concrete anchor studs causing the beam to be unstable if stood on-edge.
