On or about 2/25/2016    a)  Employees were exposed to 60 ft fall hazard when repairing a roof. Employer provided fall protection harness with lanyard but provided no lifeline ropes.

On or about 2/25/2016      a)  An employee was exposed to 60 ft fall hazard working without fall protection; employer was replacing the roof of the stairway.
