29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the rotating circular serrated blade: a) On or about 05/20/16 in the shop area; employer failed to protect employees who used the electric bagel slicer from the laceration hazard of the rotating serrated blade, when the opening on the chute used to feed the bagels was 2 3/4 inches wide at a distance of 4 inches from the blade. ABATEMENT DOCUMENTATION REQUIRED
