29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews:  a)  On or about May 16, 2016, at the above addressed jobsite - employees performing masonry operations on a tubular welded framed scaffold were not protected from falling due to missing end rails along the scaffold, thereby exposing employees to fall hazards of approximately thirty (30) feet.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about May 16, 2016, at the above addressed jobsite - employees performing masonry activities on the third floor in a rehab building were exposed to falls created by the presence of unprotected sides/edges, thereby exposing employees to fall hazards of approximately twelve (12) feet.
