29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. a) On or about May 20, 2016, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. a) On or about May 20, 2016, at the above addressed jobsite, employees performing roofing operations greater than (10) feet above ground were not protected from falls, thereby exposing employees to fall hazards. Jose Caguana was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(B)(13) which was contained in OSHA inspection number 1079860, citation number 2, item number 1 and was affirmed as a final order on November 24, 2015 with respect to a workplace located at 8845 Fairfield Ln. Tinley Park, IL 60487. Jose Caguana was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(B)(13) which was contained in OSHA inspection number 992888, citation number 1, item number 1and was affirmed as a final order on October 30, 2014, with respect to a workplace located at 6149 W. 157th Street Oak Forest, IL 60452.
