29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about May 11, 2016 the employer failed to ensure that employees tasked with operating the Crown electrical powered industrial pallet jacks were trained in the safe operation of them. Employees operate the pallet jacks to pull products and fill orders at the warehouse located at 761 Bev Road, Youngstown, OH.
