29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On or about May 20, 2016: Employees working in the Geomet lab area are exposed to fire and smoke hazards. The employer did not ensure that the fire extinguisher was mounted, identified, and positioned so as not to cause injury to the employees.

On or about May 20, 2016: Employees working in the Geomet area use N95 Respirators voluntarily. The employees were not provided with proper information on the respirators to include but not limited to the limitations of the respirators.
