OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to ejection and crushing hazards from operating powered industrial trucks:     a) On or about May 20, 2016, and at times prior, employees were exposed to ejection and crushing hazards while operating the following forklifts which were not equipped with an operator restraint device (seatbelt):  (1)  Caterpillar Towmotor Model V50B,   (2)  Two rough terrain forklifts with a 6000 pound capacity    b)  On or about May 20, 2016, and at times prior, employees were exposed to ejection and crushing hazards while operating the blue Toyota forklift where the operator restraint device (seatbelt) was secured behind the operator's seat and unusable. (a)  On or about May 20, 2016, and at times prior, employees were exposed to fall hazards on stairways that were wider than 44 inches and were not equipped with handrails on enclosed sides or stair railings on open sides.  Employees are expected to carry items up and down the stairs that are stored on upper levels.  Stairways in the following locations were missing handrails or stair railings:  1)  The stairway in storage room 11 was missing hand rails on both sides. 2)  The stairway in the Aluminum Wheel Building was missing a stair railing on the south side and a handrail on the north side.
