29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Worksite: In the following instances where employees were re-shingling roofs, the employer did not ensure that employees were adequately protected against falls to the ground below by guardrail systems, safety net systems, or the proper use of personal fall arrest systems:  1) Miller Street, Bldg. 43: An employee straddling the corner of a roof was not protected from falls in excess of 10 feet to the ground below by the proper use of a harness and lanyard system.  The worker wore a harness but was not connected to a lanyard that would stop a fall to the ground below;  2) Corner of Garfield and Miller Streets, Building 18A: Two employees conducting roofing activities were not protected from falls in excess of 10 feet above ground level.  The employees wore harnesses but these were not connected to lanyards that would stop a fall to the ground below.
