29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Access road to equipment storage area - On or about 5-20-16 - The vertical side walls of the ten foot wide roadway that was excavated for access to the pit area were not protected from cave-ins by shielding, benching or sloping of the 40 foot high walls.
