29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) On or about May 23, 2016, Stone Byron Masonry, LLC., did not ensure that each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports. A three-tier scaffold in use by workers was not fully planked, exposing employees to falls in excess of 15 feet. 29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:    (a) On or about May 23, 2016, Stone Byron Masonry, LLC., did not ensure that portable ladders, hook-on ladders, or other proper means of access was used to reach a scaffold platform. Workers climbed a three-tier scaffold using the side frames and the cross braces, exposing employees to falls of up to 15 feet. A three-tier scaffold in use by employees was not inspected for visible defects before each shift. Scaffold did not have properly installed guardrails, exposing employees to falls in excess of 15 feet.
