29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: Brillion Ironworks Inc. facility substation in Brillion, WI: On and before May 20, 2016, employees performing maintenance work on the substation transformers were exposed to arc flash hazards while pulling fuses and grounding the 12.5KV switchgear when the appropriate level of arc flash PPE was not specified or worn.
