On or about May 15, 2016, the employer did not use at least one of the measures required to prevent encroachment or contact with the power lines while disassembling the crane, exposing employees to the hazard of electrical shock.  Note: The additional measures are: i) Use a dedicated spotter who is in continuous contact with the equipment operator.  ii) A proximity alarm set to give the operator sufficient warning to prevent encroachment. iii) A device that automatically warns the operator when to stop movement, such as a range control warning device.  iv) A device that automatically limits range of movement, set to prevent encroachment. v) An elevated warning line, barricade, or line of signs, in view of the operator, equipped with flags or similar high-visibility markings.

29 CFR 1926.1407(d): Assembly/disassembly inside Table A clearance prohibited. Part of a crane/derrick, load line, or load (including rigging and lifting accessories), whether partially or fully assembled, was closer than the minimum approach distance under Table A (see �� 1926.1408) to a power line.  On or about May 15, 2016, employees attempted to disassemble the 275 ton crane to place the boom onto a flatbed truck parked below a 14,400 volt energized power lines. The load line came in contact with the power line, exposing employees to the hazards of electrical shock.  Note: Disassembling cranes under power lines is prohibited under 1926.1407(c) of this section.
