29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about 5/19/16, in the Suction Shop Parts Storage area, the employer allowed employees to lay a 1700 lbs. bearing housing over on its side on a pallet using the Clark C15CL powered industrial truck (serial #C152L0186/9669, company ID 198620), and the weight and configuration of the housing caused one end of the load attachment strap holding the housing to slip off of the forklift tine, releasing the housing to fall onto the employee squatting at the housing; an employee suffered a shattered knee cap and multiple bones around the knee cap, and a broken ankle.
