29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about May 31, 2016, at a job-site located at 40 North Ave, Tallmadge, Ohio 44278; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth of approximately 6 feet. The trench was determined to have type "C" soil.
