29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person:      On or about May 25, 2016, employees were exposed to a fall hazard 16 feet in height to paved ground level below while laying stone on an exterior wall from a scaffold that was not assembled under the supervision of a competent person and lacked a ladder for access and guardrails for fall protection.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:      On or about May 25, 2016, an employee was exposed to fall hazards greater than 10 feet in height to unpaved ground level below when working from the extension brackets on a scaffold that did not have guardrails installed on the sides, while laying stone on an exterior wall.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces and side brackets were used as a means of access:       On or about May 25, 2016, employee(s) were exposed to falls 16 feet in height to the ground level below on the extension brackets of the scaffold, which did not have a ladder for access and egress to and from the scaffold platforms, while laying stone on an exterior wall.    The Arturo Oberg was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 963536, citation number 1, item number 2 and was affirmed as a final order on 06/09/2014, with respect to a workplace located at 2134 West Wildwood, San Antonio, TX. 78201.
