29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about May 25, 2016, the employer failed to ensure that the ID grinding lathe (Heald, serial #16550/21168) had adequate guarding in place on the release swtich lever which would prevent an employee from getting his hand or finger into the moving components of the lathe.  An employee was moving the release switch lever up to allow the product to be removed from the lathe and the employee sustained an amputation of his LEFT index finger tip to the first knuckle when his finger slid past the lever and into the moving parts of the lathe.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   On or about May 25, 2016, the employer failed to report the amputation of the left index finger down to the first joint of an employee who was working on the Heald ID lathe grinding machine.  The amputation occurred on May 10, 2016 and was reported to OSHA nine (9) days post incident.
