29 CFR 1910.261(l)(9)(i): The nipping points of all drum winders and re-winders located on the operator's side were not guarded by either automatic or manually operated barrier guards of sufficient height to protect fully anyone working around them. The barrier guard was not interlocked with the drive mechanism to prevent operating above jog speed without the guard in place: (a) Plant site - On or about May 19, 2016 employees entered into Amicamatrix paper machine to re-thread the system and the machine was operated by another employees using the jog control on the outside of the machine exposing employees to unguarded in going nip points in violation of manufacturers operating procedures and by-passing entrance interlock. The employees did not use the safety jog located inside the machine which moves the rolls at a much slower rate and no presence sensing devices were in place.
