29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:  a) On or about 5/12/16 at a masonry renovation project located at 81 Greene Street, New York, New York; employees on a tubular frame scaffold did not have a correctly installed midrail in place. The cross brace intersected at 47" and provided for a toprail. The scaffold planks were placed at the same height as the intersection of the cross brace which left a missing midrail.  NO ABATEMENT CERTIFICATION REQUIRED
