29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) On or about May 25, 2016 at 8101 Airport Blvd, Mobile; The employer failed to ensure an employee using a grinder to cut rebar wore eye protection which exposed him to the hazards associated with getting particles in the eyes.
