29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. At the site on or about May 9, 2016; employees worked from a scaffold where the space between the platform planks and uprights was greater than 1 inch, exposing employees to fall hazards.

29 CFR 1926.452(c)(5)(i): Brackets used to support cantilevered loads were not seated with side-brackets parallel to the frames and end-brackets at 90 degrees to the frames; At the site on or about Mary 9, 2016; employees placed masonry rock on top of a side bracket that was not installed at 90 degrees from the scaffold frame, exposing employees to struck by hazards.
