(a)  Front of the house - On or about May 24, 2016 employees were exposed to a struck by hazard while passing and plywood without wearing hardhats.  None of these employees were wearing hardhats.

(a)     All around the house - On or about May 24, 2016 employees were exposed to a flying object hazard while using nail guns.  None of these employees were wearing eye protection.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    (a)  Roof of the house - On or about May 24, 2016 two employees were exposed to a fall hazard while working on a steep roof with no fall protection.  One of these employees was 27 feet above the ground.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  (a) Front of the house - On or about May 24, 2016 two employees were exposed to a fall hazard due to the ladder not being extended 3 feet over the landing or being tied to the building. This ladder was used to access a roof 10 feet to the landing.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  (a)  Daniel Estrada-Granados - On or about May 24, 2016  the employer did not train each employee exposed to various hazards associated with construction on how to recognize and minimize these hazards. The employer did not provide training to help employees avoid unnecessary injuries on this construction site.  The employer stated he did not provide any construction safety training to his employees. Employees were not trained to wear PPE such as: hardhats, safety glasses, and harnesses. Also employees did not know ladders must be extended 3 feet over the landing.
