29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a) 1533 Union Avenue, Pennsauken NJ - Throughout the production Area: The energy control procedures were not specific for each individual type of machine (machines include but are not limited to: cartoning machines, packaging machines, conveyor belts, etc.). The procedures did not clearly and specifically identify the placement, removal or transfer of Lockout /Tagout devices on each individual type of machine and did not specify the requirements for testing to verify the effectiveness of LO/TO devices and other control measures. Observed on or about 05/26/16. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Condition observed on or about 05/26/16. b) In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980, 1533 Union Avenue, Pennsauken NJ: Extension cords were used, as they ran under a piece of cardboard on the ground, to power the 3M taping machine, conveyor belt, paragon printer, etc. Condition observed on or about 05/26/16.
