The procedures did not clearly and specifically identify the placement, removal or transfer of Lockout /Tagout devices on each individual type of machine and did not specify the requirements for testing to verify the effectiveness of LO/TO devices and other control measures. Observed on or about 05/26/16. 29 CFR 1910.305(g)(1)(iv)(A): Flexible cords were used as a substitute for the fixed wiring of a structure;    a)  In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980,  1533 Union Avenue, Pennsauken NJ: Extension cords, strung from a wall to a piece of equipment approximately 68 inches above the ground surface below, were used on a permanent basis to power a printer as well as other equipment.  Condition observed on or about 05/26/16.    b)  In the area of the CRT-02 Cartoner Machine - Model #3400V - Serial Number E3400V - SA02A000980, 1533 Union Avenue, Pennsauken NJ: Extension cords were used, as they ran under a piece of cardboard on the ground, to power the 3M taping machine, conveyor belt, paragon printer, etc. Condition observed on or about 05/26/16.
