29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a. Grinding Area Grinder D433 Left Side: On or about March 26, 2016 and times prior employees used a 10 inch abrasive bench grinder to grind ,drill bits and lathe bits. The adjustable tongue guard was not adjusted on the left side resulting in a 3/4 inch opening between the wheel and the tongue guard. b. Grinding Area, Grinder D433 Right Side: On or about March 26, 2016 and times prior employees used a 10 inch abrasive bench grinder to grind drill bits and lathe bits. The adjustable tongue guard was not adjusted on the right side resulting in a 1 1/2 inch opening between the wheel and the tongue guard. c. Grinding Area, Dayton Grinder D438 Left Side: On or about March 26, 2016 and times prior employees used a Dayton 10 inch abrasive bench grinder to grind drill bits and lathe bits. The adjustable tongue guard was not adjusted on the left side resulting in a 1 inch opening between the wheel and the tongue guard. d. Grinding Area, Dayton Grinder D438 Right Side: On or about March 26, 2016 and times prior employees used a Dayton 10 inch abrasive bench grinder to grind drill bits and lathe bits. The adjustable tongue guard was not adjusted on the right side resulting in a 1 1/2 inch opening between the wheel and the tongue guard.
