29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.  a) Jobsite - The employer did not ensure that all working levels of the supported tubular welded frame scaffold were fully planked or decked between the front uprights and the guardrail supports.  Employees were performing stucco operations from the 2nd and 3rd working platforms, approximately 15 and 21 feet above ground level.   Noted on or about 5/25/16.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) Jobsite: Employees conducting masonry operations from a supported tubular welded frame scaffold, from platforms approximately 15 and 21 feet above ground level, were not provided with a safe means of access with the use of portable ladders, hook-on ladders or other acceptable means. Employees were observed climbing down using the rungs of the integral prefabricated access scaffold end frames.   Observed on or about 5/25/16.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they or any conductive material handled on them might come closer than 10 ft. to power lines.  a) Jobsite: The right and left sides of the supported tubular welded frame scaffold were erected within 2 feet of unguarded electrical wires exposing employees conducting masonry operations in proximity to the wires to electrical shock hazards.
