29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: (a)Acme Pallet, Inc. @ 4700 N I-90 Service Rd, Rapid City, SD 57719: On and prior to 5/19/2016, the employer did not ensure that the Saw Service table saw was guarded by an automatically adjusting hood. The employer used a block of wood to elevate the guard to allow crosscutting 2x4's. This condition exposed the employees to an amputation hazard.

29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of pallet dismantlers: (a)Acme Pallet, Inc. @ 4700 N I-90 Service Rd, Rapid City, SD 57719: On or prior to May 19, 2016, the employer did not ensure employees were protected from injury, in that, the point of operation on the pallet dismantlers was not guarded. The horizontal band saw had a guard that was provided by the manufacturer above the point of operation. The guard was removed. This condition exposed employees to amputation hazards.

29 CFR 1910.305(d)(2): Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons: (a)Acme Pallet @ 4700 I-90 Service Road, Rapid City, SD 57701: On and prior to May 19, 2016, the employer did not ensure a 208V electrical panel located in the NW corner of the facility with exposed energized circuits was covered by a dead front. The panel was missing the dead front covering and exposed energized circuits and bus bars. This exposed employees to an electrical shock hazard and electrocution hazard.
