29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: Note: Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift. Location - 14 Strawberry Hill Rd. Andover. On or about May 26, 2016, the employer did not ensure that the rough terrain forklift used to lift an employee up to 16 feet above ground was specifically designed by the manufacturer for such use.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about May 26, 2016, the employer did not ensure that fall protection was used while lifting or supporting personnel up to 16 feet with a rough terrain forklift.
