Wesex Corporation, 2755 Kirila Blvd. Hermitage, PA 16148, East wall next to spray booth areas, on or about May 13th, 2016, and times prior thereto: Employees worked with and around exhaust from spray booth operations that involved the use of flammable substances. A fire protection program had not been established for this worksite.

Wesex Corporation, 2755 Kirila Blvd. Hermitage, PA 16148, East wall next to spray booth areas, on or about May 13th, 2016, and times prior thereto: Employees worked with and around exhaust from spray booth operations that involved the use of isocyanates. Employees were not aware of the health hazards associated with isocyanates.
