On or about May 20, 2016, the employer failed to provide an employee performing painting activities from a makeshift platform with a standard railing on all open sides, thereby exposing the employee to approximately 13 foot fall to the concrete floor.

29 CFR 1910.28(a)(2): The footing or anchorage for scaffolds shall be sound, rigid, and capable of carrying the maximum intended load without settling or displacement. Unstable objects such as barrels, boxes, loose brick, or concrete blocks shall not be used to support scaffolds or planks. On or about May 20, 2016, an employee performed painting activities from a makeshift scaffold platform was exposed to a fall hazard of approximately 13 feet to the concrete below when the scaffold collapsed.

On or about May 20, 2016, an employee performed painting activities from a makeshift scaffold platform was exposed to a fall hazard of approximately 13 feet to the concrete below when the scaffold collapsed. The scaffold was not secured to a permanent structure.
