Perl, Paola KS:  An employee working inside a trench was exposed to struck-by and engulfment hazards in that training in the recognition and avoidance of hazards associated with working in trenches and excavations had not been provided.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).   At the jobsite located at 209 S. Perl, Paola KS:  An employee working inside a trench greater than 5 feet in depth was exposed to struck-by and engulfment hazards in that the trench was not equipped with a protective system.
