29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) At the jobsite, on or about 5/26/16: An employee was working approximately 20 feet from the lower level, on the formwork and outside the protection of the guardrail system, without fall protection. b) At the jobsite, on or about 5/26/16: Employees using personal fall arrest equipment when climbing the formwork and outside the protection of the guardrail systems, did so without an appropriate anchor point. Abatement documentation must be submitted for this item.
