29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) 506 Vine Street, on or about 5/27/2016: Employees were capping the ridge at the north side of the house with a pitch of 8:12 and a ground to ridge distance of approximately 40-feet. The workers removed the anchor points from the ridge and did not established a new one to be connected at all time, employees  exposed to a fall hazard.     b) 506 Vine Street, on or about 5/27/2016: Employee was installing roof shingles at the west side of the house porch area with a ground to eave distance of approximately 9-feet and the worker was not provided with any type of fall protection, employee exposed to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     a) 506 Vine Street, on or about 5/27/2016: Workers were installing the roof shingles at heights ranging from 9-20-feet above the lower working level without fall protection and fall protection training was not provided to them.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) 506 Vine Street, on or about 5/27/2016: Employees were using a ladder located at the north side of the house to access the roof with a ground to eave distance of approximately 20-feet and the ladder was not extended 3-feet above the landing surface, employees are exposed to a fall hazard.     b) 506 Vine Street, on or about 5/27/2016: Employees were using a ladder located at the west side of the house to access the porch area with a ground to eave distance of approximately 9-feet and the ladder was not extended 3-feet above the landing surface, employees are exposed to a fall hazard.
