On or about May 27, 2016 during the installation of roofing material at the job site located at 354 Adelaide Ave SE, Warren, Ohio 44483; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material. The employees were using an air driven nail gun to install shingles and roofing material on the east side of the roof.
