29 CFR 1926.1101(k)(2)(i): Before work subject to this standard was begun, building and facility owners did not determine the presence, location, and quantity of ACM and/or PACM at the work site pursuant to paragraph (k)(1) of this section: a) First Floor Office Areas - On or about May 10,2016, the employer did not conduct a comprehensive asbestos survey in order to determine the presence, location, and quantity of ACM and/or PACM prior to work being started. Only a limited asbestos survey was conducted which did not reveal that floor tiles contained 3.4% to 4.5% chrysotile asbestos. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Throughout Facility - On or about 5/20/16, the employer did not provide information and training to employees who were exposed to hazardous materials such as, but not limited to drywall dust (from wall demo work) containing limestone, a respiratory irritant, silica (during grinding of concrete floors), and possible asbestos (from ACM containing floor tile that was disturbed) all from construction activity on going in the building. Employees were not informed of these hazards that they were being exposed to from the construction activity in the building. ABATEMENT CERTIFICATION REQUIRED
