29 CFR 1910.134(f)(2): The employer did not ensure that employees using tight fitting face piece respirators pass an appropriate qualitative or quantitative fit test prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. (a) On or about 5/31/2016, at the site, for the carpenter and laborer wearing a 3M N95 respirator while grinding concrete. The employees were not fit tested, either qualitatively or quantitatively, prior to being required to use this respirator by the employer.
