Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat.  a) Chemical Storage Room, on or about May 27, 2016 or prior thereto:   Employees were exposed to the hazard of an abrasive grinding operation performed approximately three feet from a closed container of a Category 3 Flammable Liquid.  A Hay Hook was being dulled with a Clarke MT 1205A, S/N 040601723, 4 1/2" Hand Held Angle Grinder at 11,000 RPM, while approximately three feet from a closed container of Minwax Polyshade Stain.    The Material Safety Data Sheet of the Minwax stain states that, "closed containers may explode when exposed to extreme heat."    The Minwax stain has a Flash Point of 119 degrees Fahrenheit.   NOTE:  Abatement certification and documentation are required for this item.
