(a) North side building on roof - On or about June 1, 2016 employees nailing sheets of plywood to face of building using a pneumatic air gun were not wearing ANSI Z87.1-1968 safety eye glasses for protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) North side of building -  On or about June 1, 2016 employees working on low sloped roof framing window gained access using a closed 6 foot step ladder that was leaning against the building and did not extend 3 feet past the edge of roof 10 feet high .

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  (a) North side building - On or about June 1, 2016 the employer had not provided retraining to employees stepping on the top plate of a 6 foot fiberglass ladder to gain access to the roof.   The step ladder was folded and leaning against the building.  This demonstrated that employees lacked the understanding and knowledge of the proper construction, use, placement, and care in the handling of all ladders used on job site.
