The disconnect knife switch was in the ON position and was not reachable from any side.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment or walls) was not protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to [prevent tools or materials from falling through: Lemoore Naval Air Station, Exchange Store Warehouse, Building 796, on 05/26/2016: The second level storage area floor is comprised of metal grates. The metal grates have openings more than one inch in width allowing small objects such as household goods, personal items and beauty products to fall through the openings onto employees working below.
