29 CFR 1910.178(o)(1): Loads which were not stable or safely arranged were handled:  a) Laminator Area:  Employees were attempting to move a heavy pressure/tension roller off of the laminator machines using two powered industrial trucks equipped with two double-basketed synthetic slings and the roller was not stabilized, causing the sling and the roller to slip off the forks and fall, exposing employees to an amputation injury, on or about December 11, 2015. 29 CFR 1910.184(c)(5): The load was not balanced to prevent slippage of the sling, when used in a basket hitch:  a) Laminator Area: Employees were using two synthetic slings in a basket hitch on the forks of two forklifts to move a heavy pressure/tension roller off of the laminator machine and the load was not balanced to prevent slippage of the sling, causing the sling to slip off the forks and expose employees to a serious injury, on or about December 11, 2015.  Abatement Certification Required.
