Violation occurred on or about May 24, 2016

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface: a) Annex Building, 163 Madison Avenue, Irvington, NJ Subcontractor employees removing roofing asbestos on the 1st floor roof section were exposed to fall hazards of approximately 10 feet, in that the warning lines around the perimeter of the roof were sagging from the stations to approximately 24 inches from the roof surface. Violation occurred on or about May 24, 2016
