29 CFR 1926.501(b)(1): Each employee on a working/walking surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet ( 1.8m ) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On June 2, 2016, on the 2nd floor near the door, there was a floor opening that was 13 feet long and 4 feet wide that did not have an adequate fall protection system in that the opening had caution tape instead of a guardrail, exposing employees to a 10 feet fall hazard.
