29 CFR 1926.20(b)(2): The employer did not initiate and maintain a program of frequent and regular inspections made by competent persons at the jobsite: a) Safety inspection was insufficient; an employee was struck by a drill rig pipe being lifted by an excavator machine when the pipe slipped from the yellow nylon sling. The employer did not ensure a safe distance is maintained between the machine and employees on foot; and to ensure slings are rigged securely to the load being lifted. Location: 122 E 23 Street, New York, N.Y. On or about 5/25/2016 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
