29 CFR 1910.1030(d)(4)(iii)(A)(1): Contaminated sharps were not immediately discarded in containers that were closable, puncture resistant, leak proof and labeled or color-coded according paragraph g(1)(i) of this standard: On or about April 24, 2016, the employer did not ensure that contaminated sharps were immediately discarded in appropriate sharps containers. Employees were exposed to the hazard of being stuck by contaminated sharps.

29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable. On or about June 7, 2016, and times prior thereto, the floors of the following shower/whirlpool rooms were not kept dry where employees walk and push residents in shower chairs. Mats are used in showers but do not protect the rest of the floor in the room. a) West Hall b) North Hall c) East Hall Employees were exposed to the hazard of slipping on wet floors.
