29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe. a) Worksite was located at 801 Jersey Avenue, Jersey City, NJ 07310 Employees were exposed to struck by hazards as a generator was lifted to the roof by a crane using a sling that was badly worn in excess of allowable limits. Violation was observed on or about 5/4/2016.

29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type was engaged to each other such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. a) Worksite was located at 801 Jersey Avenue, Jersey City, NJ 07310 An employee was exposed to fall hazards as he was attached by an open to an open connection while he was using a 6 foot lanyard connected to a 30 foot self retracting lifeline while working from the roof of the building, 21 stories above the ground. Violation was observed on or about 5/4/2016.
