29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about June 3, 2016, the employer failed to ensure that an employee tasked with performing Lock Out Tag Out on equipment and machinery was trained as an authorized Employee. An employee performed LOTO on Press #4 to work on the air pressure component of the printing press.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about June 3, 2016, the employer failed to provide affective guarding on the 7 automatic carousel printing presses located in the second building upper floor. Two of the pallets (clothing pallet) are exposed during each cycle to allow employees to put a shirt onto each pallet. Then the carousel is activated and the two exposed pallet arms rotate into the press and another two become exposed to repeat the process. The width for the whole opening during this time is approximately 94 inches.
