29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about May 31, 2016, the employer did not ensure the door latches on the Dual Wichter machine, which prevents access to the machine point of operation was affixed so that it could not be easily opened. 29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:    (a) On or about May 31, 2016, the employer did not ensure that the three press machines shaft ends where the ink is distributed were guarded.  The shaft ends on the press machines were measured at two and one half inch in length and the diameter was approximately one inch.        Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   (a) On or about May 31, 2016, the employer exposed employees to an electrical hazards due to missing electrical faceplate. The faceplate cover in the administration office was missing and an electrical heater was plugged into the electrical outlet.
