29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered and brakes set to prevent movement.  Worksite: The load engaging means of the JLG all-terrain forklift (PIT) was not fully lowered and the brakes were not set to prevent movement prior to the operator dismounting from the PIT.  On or about June 03, 2016, the above PIT rolled/moved back-ward and ran into an employee after the operator dismounted from the PIT, and the employee sustained serious injuries from the incident.
