While the cover is not in place, the floor hole was not constantly attended by someone or was not protected by a removable standard railing.  a) At the sorter, on or about and at times prior to May 23, 2016, employees accessed the decking of the sorter, by crossing an open gap that measured 10 and a half inches across, to loosen cants that became stuck and as such were exposed to the hazards associated with broken bones, sprains, lacerations, and bruises.
