29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. A. James J. Campbell Construction LLC worksite located at One Courthouse Square, Bryan, Ohio: On or about May 31, 2016, the employer did not ensure employees were protected from fall hazards while working from the supported scaffold. The employees were exposed to fall distance of approximately 145.00' due to missing guardrail located in multiple areas. No form of fall protection was in place.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. A. James J. Campbell Construction LLC worksite located at One Courthouse Square, Bryan, Ohio: On or about May 31, 2016, the employer did not ensure employees were protected from falling approximately 148.00 feet while transitioning from the scaffold system into the structure. The exposed employees were entering the structure through a window opening located on the East side of the structure.
