29 CFR 1910.266(h)(1)(vi): Each danger tree, including lodged trees and snags, were not felled or removed using mechanical or other techniques that minimize employee exposure before work is commenced in the area of the danger tree. Black Springs Campground, Stanislaus National Forest - On 5/13/16 the top portion of a felled danger/hazard tree (28" diameter white fir), became hung up when it missed the intended lay. This resulted in the top portion of the tree to be suspended approximately 20 feet above ground and under extreme bind between four green trees. The bucking techniques employed did not minimize employee exposure to struck by hazards. A sawyer was struck by the bole and received near fatal injuries. Abatement documentation and abatement are required.

29 CFR 1910.132(d)(1): Hazard assessment and equipment selection. The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). Written Hazard Assessment - On 5/17/16, it was noted that the written hazard assessment was inadequate because it did not list all types of required personal protective equipment (PPE) for tree felling operations such long sleeved shirts and pants, axes, and wedges. It should be noted that employees have been provided the required PPE.
